Case Management Activity
I would argue, that case management is the most important metric of the Recovery Navigator Program. According to the Uniform Program Standards:
Individuals referred to RNP staff have a multitude of needs which must be addressed to achieve stabilization and set the stage to address problematic activities associated with their quality of life. Case managers work to address the participant’s social determinants of health, including legal advocacy and access to a stable legal income stream. Intensive case management provides increased support in accessing these services and assistance in many aspects of the participant’s life. Case management is the “golden thread” that stays with the participant over time and works to address setbacks and barriers.
This is the reason why case management is considered the "Golden Thread" and an important metric to consider when evaluating the program.
Case Management is a separate piece of RNP operations, where Referrals and Outreach deals in engagements, the ideal end result of engagements is towards case management and ideally, better outcomes for enrolled clients.
If we look at the data between Gather Church and Destination Hope & Recovery, there are a few things to note:
- Gather Church had much more recorded case management activity during their operation (9 months vs 7 months of data)
- Gather Church had much more recorded case management activity per month on average (140) than DHR (49)
- Gather Church recorded more intensive case management activity than DHR
One concern that I have is regarding the case management client handoff during the transition between Gather Church and Destination Hope & Recovery. According to the monthly reports, Gather Church transitioned their case management load to DHR. This means that DHR didn't have to start from scratch and use engagements to drive case management. In short, they inherited all of Gather's former case management clients. Given the differences between the case management activity, that DHR has so much lower recorded activity, it may seem to indicate that DHR is not as actively engaged with case management as Gather had been.
