By Screening/Case Management Level

Part of the outreach process for RNP, is to screen individuals and determine what level of care they need based on multiple factors. A screening designation of "Outreach Status" is in short, the default status of a person. If an individual decides they wish to work toward a goal or have someone case manage them, they will be designated for either light or intensive case management. Both light or intensive case management requires a signed consent or release of information (ROI) respectively.

According to Uniform Program Standards:

A brief wellbeing screening will be administered by outreach and referral staff members. The wellbeing screening will consider the immediate biological, psychological, and sociological needs of the individual being referred. The individual will become a program participant if they are interested in case management and ongoing support.
